Stomatitis Infections
Candidosis
describes a group of yeastlike fungal infections involving the
skin and mucous membranes. Infection is caused by Candida
species, typically, Candida albicans. Patients who wear
dentures are prone to episodes of candida stomatitis as the
balance of the normal oral flora is disturbed by the presence
of plastic dentures.
By
tradition, the most commonly used classification of oral
candidosis divides the infection into 4 types including (1)
acute pseudomembranous candidosis (thrush), (2) acute atrophic
(erythematous) candidosis, (3) chronic hyperplastic candidosis,
and (4) chronic atrophic (erythematous) candidosis.
Thrush
may be observed in healthy neonates or in persons in whom
antibiotics, corticosteroids, or xerostomia disturb the oral
microflora. Oropharyngeal thrush occasionally complicates the
use of corticosteroid inhalers. Immune defects, especially HIV
infection, immunosuppressive treatment, leukemias, lymphomas,
cancer, and diabetes, may predispose patients to candidal
infection.
Erythematous
candidosis may cause a sore red mouth, especially of the
tongue, in patients taking broad-spectrum antimicrobials. It
also may be a feature of HIV disease. Median rhomboid
glossitis is a red patch occurring in the middle of the dorsum
in the posterior area of the anterior two thirds of the tongue
and especially is observed in smokers and in those with HIV
disease.
Chronic
mucocutaneous candidosis (CMC) describes a group of rare
syndromes, which sometimes include a definable immune defect,
in which persistent mucocutaneous candidosis responds poorly
to topical treatment. Generally, the more severe the
candidosis, the greater the likelihood that immunologic
defects (particularly of cell-mediated immunity) can be
identified.
Further,
C albicans is a harmless commensal organism inhabiting the
mouths of almost 50% of the population; however, under
suitable circumstances, it can become an opportunistic
pathogen. A suitable circumstance may be a disturbance in the
oral flora or a decrease in the immune defenses.

Treatments
The management of
patients with oral candidiasis is twofold: (1) to identify,
correct, or eliminate predisposing or precipitating causes and
(2) to provide antifungal therapy.
The underlying systemic conditions (such as diabetes,
malnutrition, and anemia) and the discontinuation of
broad-spectrum antibiotics are recommended for the first
approaches. Local resistance can be improved by good oral
hygiene and by leaving dentures out as much as possible.
The six drugs that are chiefly used for antifungal therapy are
gentian violet, nvstatin, amphotericin B, miconazole,
clotrimazole, and ketoconazole. Nystatin and amphotericin have
been the standard drugs used for oral candidal infections for
the last 35 years. Each of these drugs is absorbed poorly from
the gastrointestinal tract but is excellent for topical use on
mucous membrane and skin lesions. Most dentists and physicians
stress the importance of continuing antifungal therapy at
least 2 weeks following disappearance of signs and symptoms of
oral lesions.

Remedies for Thrush (Candida Albicans)
in Infants & Others
Thrush is a yeast infection in
the mouth. A tongue that’s coated white and/or pockets of
white yeast in the cheeks or the corners of her mouth are good
indicators of a thrush infection. The insides of one's cheeks
and throat may also look redder than normal.

Yogurt
Remedy
- Besides its great taste, eating yogurt is also one of the
better known home remedies for adults with thrush that is
worth trying. For infants, offer a fingerful of yogurt to be
sucked, or smear it on the yeast patches in baby’s mouth,
and on the breastfeeding mother's nipples using a cotton swab
(do clean your nipples of yogurt before nursing again).
Discard the swab or wash your hands after applying the yogurt,
taking care not to contaminate your yogurt with thrush.

Baking
Soda / Vinegar Remedy
- Make
a solution of one-teaspoon baking soda in eight ounces of
water. Use a cotton swab to wipe the solution on the insides
of your cheeks, gums and tongue after after eating. Prepare a
fresh solution every day, and stir well before using.
Again, for nursing mothers, discourage thrush on your nipples
with a vinegar solution of one-tablespoon vinegar in a cup of
water. (You need not make a fresh solution of this every day).

Plantain Seed Remedy
- Soak
the plantain seeds in enough water to cover them and soak them
until they swell which may take several hours or overnight.
This will make a gel-like substance that you can put on the
patches of thrush. Plantain seeds are easily gathered in the
fall, but you can also buy them as psyllium seeds.
It has been suggested that thrush can clear up within three
days, but it is recommended you continue with the above
treatments for several weeks to completely clear the thrush
and prevent reoccurrence.
While the thrush persists make it a habit to wash your hands
often. For best results for moms, wash your hands every time
you touch the baby’s mouth or your nipples. It can be pretty
unrealistic and frustrating, so just do your best and try not
to let the home remedy for thrush be more of a cause for
stress, as that just feeds the infection.
· Acidophilus
Remedy
- The
first is breaking open acidophilus tablets and rubbing the
powder on the pockets of thrush. Next, supplement your daily
diet with acidophilus tablets or capsules.
· Calendula
Tincture Remedy
½ cup tightly packed whole, dried calendula flowers
1 ounce distilled water
4 ounces grain alcohol, 190 proof
Grind dried flowers in a spice or coffee grinder (one not yet
used for coffee) or use a mortar and pestle. Grinding will
reduce the flowers to a powder measuring ¼ cup (about 20
grams, or a scant 1 ounce). Place the powder in a pint jar
with a tight-fitting lid. Add water to alcohol in a measuring
cup and stir. Pour this mixture of alcohol and water over the
herbs. Stir to completely dampen every particle of herb. Shake
the tincture twice a day for 14 days.
On the 15th day, let the jar sit undisturbed. On the next day,
gently pour the tincture (decant) into another container, such
as a jar or measuring cup. Press the finished calendula petals
by using either a tincture press or by emptying the herb into
a piece of cheese cloth or muslin, wrapping and pressing down
to extract all of the essence from the tinctured herb; combine
this final-pressed mixture with what has been poured off.
Filter if desired. Store in a labeled opaque glass bottle.
Approximate dose is 1 to 2 ml, three to four times a day.

Colloidal Silver Treatment
* Adults--4 dropperfuls once daily, or more, as needed.
* Children--1 or 2 dropperfuls once daily.
* May be taken with distilled water or pure juice at least two
hours before or after a meal.
* May be applied topically.
NOTE: Consuming too much colloidal silver, over time, can
result in a permanent graying of the skin - a condition known
as argyria.

Definitions
(For Those of Us Who are Medically Challenged)
Nystatin - It
has an unpleasant taste and may cause nausea and GI
disturbance. Vaginal tablets in combination with unsweetened
mints or chewing gum are better tolerated.
Clotrimazole -
It is more palatable than nystatin but contains dextrose which
may promote dental caries.
Fluconazole -
Fluconazole is superior to nystatin, clotrimazole, and
ketoconazole. High doses (up to 800mg/day) can be used in
difficult cases.
Itraconazole -
The capsules have limited bioavailability and their absorption
is improved if they are taken with a fatty meal. The efficacy
of the capsules is thought about equal to that of ketoconazole.
Ketoconazole -
This agent has limited bioavailability, requires an acidic
environment for best absorption, and causes liver toxicity. As
it is less efficacious than fluconazole and itraconazole
suspension, it is less frequently used.
